Oroville Hospital Post Acute Center provides skilled nursing and rehabilitation services to those who require medical, nursing, and/or rehabilitative care. Our programs and services are developed and implemented in a manner that promotes resident choice, dignity, and well-being. We respect our residents’ rights and encourage their participation in their plan of care.
Our facility is equipped to meet a variety of healthcare needs, ranging from short-term rehabilitation to long-term care. Services include skilled nursing, rehabilitation therapy, post-acute care, social services, and much more.
Skilled Nursing Services
Skilled nursing is needed when one requires a licensed nurse to manage, observe, or evaluate their care. Our highly trained, compassionate nurses work with each resident, individually, to help them achieve overall health and success.
Each resident comes to us with different, and specific, rehabilitation needs. Our rehabilitation staff is committed to creating individualized therapy programs to address these individual needs in order to help each resident attain their highest functional ability.
Post-acute care is provided to those who are recovering from illness or suffering from a chronic illness. These residents most often come to us from a hospital, and we work with a range of medical care services to continue their recovery. Our post-acute care program helps to improve the transition from hospital to home.
Our social services team works with each resident to share essential information, manage requests, help in care planning, and help manage transitions. Each member of our social services team is compassionate and seeks to be aware of the individual needs of each resident.
We provide regular nutritional assessments from our registered dietitian. Our nutrition experts will work with you to individualize your diet based on your nutritional needs. This personalized approach promotes your ongoing health and independence.
